The 2002–03 Macedonian Second Football League was the eleventh season since its establishment. It began on 10 August 2002 and ended on 7 June 2003.[1]

For deciding promotion, relegation and promotion/relegation play-offs: 1st points; 2nd head-to-head points; 3rd head-to-head goal difference; 4th head-to-head goals scored; 5th play-off
1Pobeda Valandovo was taken out of competition after round 15. All matches annulled.
(C) = Champion; (R) = Relegated; (P) = Promoted; (E) = Eliminated; (O) = Play-off winner; (A) = Advances to a further round.
Head-to-Head: used when head-to-head record is used to rank tied teams.
