EQ-5D

The EQ-5D is a widely-used survey instrument for describing health-related quality of life states.[1] It is one of several such instruments that can be used to determine the quality-adjusted life years associated with a health state. The name is derived from the survey methodology, which measures quality of life in five dimensions and was developed by the EuroQol Research foundation.[2]
