Mayhem was a professional wrestling pay-per-view (PPV) produced by World Championship Wrestling (WCW) that replaced the promotion's November PPV event World War 3 which was held from 1995 to 1998 and was held in the month of November in 1999 and 2000. It is noted for being the first wrestling pay-per-view named after a video game, rather than the video game named after a pay-per-view. The 1999 match between Bret Hart vs Chris Benoit was featured in the opening credits of the TV Show Malcolm in the Middle. The rights to the event is now owned by the WWE since 2001.

World Heavyweight Championship Tournament Semi-Final: Chris Benoit defeated Jeff Jarrett (9:27)
- Benoit pinned Jarrett after hitting him with his own guitar.
Evan Karagias (with Madusa) defeated Disco Inferno (with Tony Marinara) to win the WCW Cruiserweight Championship (8:28)
- Karagias pinned Inferno with a diving crossbody.
Hardcore Championship Tournament Final: Norman Smiley defeated Brian Knobs (with Jimmy Hart) to become the first WCW Hardcore Champion (7:27)
- Smiley pinned Knobs after Hart accidentally hit Knobs with a garbage can.
The Revolution (Perry Saturn, Dean Malenko and Asya) (with Shane Douglas) defeated The Filthy Animals (Eddy Guerrero, Billy Kidman and Torrie Wilson) (with Konnan) in an Elimination match (10:55)
- Malenko pinned Kidman with a roll-up (2:56)
- Guerrero pinned Malenko with a Hurricanrana (5:03)
- Guerrero pinned Asya after a Frog Splash (6:28)
- Saturn forced Guerrero to submit with the Rings of Saturn (10:17)
- Saturn pinned Wilson after a low blow (10:55)
Buff Bagwell defeated Curt Hennig in a Retirement match (7:47)
- Bagwell pinned Hennig after a Buff Blockbuster.
World Heavyweight Championship Tournament Semi-Final: Bret Hart defeated Sting (9:27)
- After The Total Package interfered by attacking Sting with a baseball bat, Hart was announced as the winner via disqualification. Not wanting to win in this manner, Hart demanded the match be restarted, and ultimately forced Sting to submit with the Sharpshooter.
Vampiro (with Jerry Only) defeated Berlyn (with The Wall) in a Dog Collar match (4:57)
- Vampiro forced Berlyn to submit with a Camel Clutch.
Meng defeated The Total Package (with Elizabeth) (5:23)
- Meng pinned Package with the Tongan Death Grip.
Scott Hall defeated Booker T to retain the WCW United States Championship and WCW World Television Championship (6:04)
- Hall pinned Booker after an Outsider's Edge.
- Hall was originally supposed to face Rick Steiner for his World Television Championship, but Steiner was too injured to compete. Hall was awarded the title by forfeit and Booker T was named as his replacement.
David Flair fought Kimberly Page to a no contest (4:55)
- The match was declared a no-contest after Chris Kanyon, Diamond Dallas Page, and Arn Anderson all interfered in the match.
Goldberg defeated Sid Vicious in an "I Quit" match (5:30)
- The referee ended the match after Goldberg put Vicious in a Cobra Clutch.

World Heavyweight Championship Tournament Final: Bret Hart defeated Chris Benoit to win the vacant WCW World Heavyweight Championship (17:44)
- Hart forced Benoit to submit with the Sharpshooter.
