Mayhem Festival 2012 is the fifth annual Mayhem Festival. Dates were announced on October 26, 2011.[1]

On June 7, 2012 Mayhem officially announced that High on Fire had pulled out of the festival, due to lead singer Matt Pike entering treatment for alcohol rehabilitation.[1][2]
