Casey first appeared in the fourth season of the series. Though a little flaky, he quickly wins over the newly single Donna Pinciotti (Laura Prepon) and they start dating, much to the chagrin of Donna's former boyfriend Eric Forman, whom Casey calls "Foreplay." Eric openly objects in light of how Kelso's relationship with Jackie was in perpetual Hell, something he didn't want to see Donna fall into. Further motivation is provided when Michael, spurred on by having read Cosmopolitan, seriously warns Eric that Casey will just use and abuse Donna.
Charming, athletic and handsome, Donna thought Casey was a real prince charming and the best boyfriend a girl could ask for—she even believed him when he said he loved her. Moments after Casey's declaration, he secretly reveals to Eric that it was all 'just words'.
Eric tries to warn Donna that this was a mistake, but Donna blithley ignored him. Later when Donna was seen drunk in the middle of a school day with Casey at the Hub, Kitty got together with Donna's dad Bob and Red to put an end to their relationship. When push came to shove, with Eric overseeing the proceedings, Casey quickly gave in and agreed to break up with Donna—rather, dumping her in front of her friends and family. Casey lazily explains to the shocked and confused Donna that their relationship had gotten to be too much of a hassle, and lazily admitted that he really never loved her to begin with.
The shock of her sudden realization of Casey's true nature along with her own poor judgment and over confidence, coupled with being subsequently rejected by Eric, who refused to be her "second choice" left Donna in so much emotional turmoil that she fled to California to crash with her mother. Eric later came to California looking for her and he and Donna reunited as a couple.
Casey and his brother Michael are constantly fighting. For example, Casey once locked up Michael in a stinky closet full of dead fish to make him cry. Michael retaliates by spitting in Casey's socks while he's gone or sleeping naked in his bed after a couple of days without taking a shower.
Casey holds Point Place High's Rushing Record for football. He also is famous for his Trans-Am. When Donna says that she loves the Trans-Am Casey replies "everybody does" referring to other girls.
In Season 4, it is revealed that Casey was once in the Army, (which he explained to Red Forman on "Everybody Loves Casey"). He says that he left because "it was like they were always trying to tell him what to do". In "Everybody Loves Casey" he is shown to always have a knife on hand and always keeps an extra supply of beer in the trunk, and that he learned this tip in the military.
Although Eric extremely dislikes Casey, his family takes a liking to him. Red hits it off well with Casey after he gives him an extra case of beer for his party after the "idiot neighbors drank all the beer" during a barbecue. Kitty likes him as well, and is very happy when he is around, and makes no effort to hide the fact that she has a crush on him, even after he becomes a bad influence on Donna and reveals to her that he works at the strip club (as she sees him there, she exclaims "Oh my God, Casey!" in an excited way while Donna exclaims this in a surprised and angered way).
He reappears in the season 6 episode "My Wife", when he climbs out of the window of Eric and Donna's neighbor after sleeping with a married woman. Later, Donna and Jackie discover Casey works at a strip club, when they take Kitty out for Donna's bachelorette party. This same night, he stays at the Formans', pretending to be sleeping in Eric's room while in fact sleeping with Laurie.
He returns in episode 21 of season 7 ("2120 So. Michigan Ave"). Eric had not graduated high school, technically, because he had never passed gym. He takes a session of summer school gym, unfortunately Casey is the teacher. After multiple confrontations, Casey agrees to pass Eric if Donna will go out with him. Donna does not take this well, although Eric does--which Donna takes even worse. He then agrees to pass Eric if the latter is able to perform one pull up (knowing that because of his lack of strength and skinny muscles, this would be extremely difficult for Eric). Growing tired of Casey's insults as well as the blackmail ultimatium, Eric does a pull up after angrily protesting to the latter that "It's not Foreplay, it's Forman!". It is hinted that after this, Casey seems to gain more respect for Eric.
