The following is a list of maintenance swaps made between the state and counties around the state according to the New York highway law. See New York State Route 361 for a reference to use in articles. Unless otherwise noted, the swaps took effect on April 1 of that year.
In both columns, the roads are listed using their designations/names immediately prior the swap. Any former designations that the road had long before the swap or designations that came about as a direct result of the swap are listed in parentheses following the name. If no parenthetical notes follow the road name, then that road was unchanged except for who maintained the roadway following the swap.
Note that there are a large number of undated highway swaps in the law text; these are not listed here but should be examined at some point. These undated swaps are ones that took place upon the completion of a roadway instead of at a set date.

- ^ a b c d NY 26 was rerouted onto its modern alignment between Carthage and Theresa as a result of the 1979 swap, replacing NY 411 from NY 37 west of Theresa to Philadelphia.
- ^ Part of the route is within Fort Drum and is neither NYSDOT or county maintained.
- ^ NY 178 was truncated to US 11 on its eastern end.

The NY 921A designation has been vacant since at least 2003, so it is possible that the route carried the NY 921A designation up until 1981.

- ^ The law text includes only the portion between NY 11B and Lavery's Corners; however, the Lavery's Corners-Lawrenceville segment had the same SH number as the NY 11B-Lavery's Corners section and appears to no longer be state maintained.

- ^ NY 941H was the northeast leg of the wye intersection between NY 245 and NY 247 at Baldwin Corners - a mere 0.04 miles. Its last route log appearance was in 2004 and its last traffic count/mileage appeared in the 2003 Ontario County traffic counts.
- ^ NY 942W is still listed in the 2006 Traffic Data Report and is listed in the 2004 route log. However, the 2007 route log no longer has the route.
