West Virginia Route 45 is an east-west state highway located in the Eastern Panhandle of West Virginia. The western terminus of the route is at the Virginia state line four miles west of Glengary, where WV 45 continues west as secondary State Route 681. The eastern terminus is at West Virginia Route 230 and West Virginia Route 480 in Shepherdstown.
