USAir Flight 405 was a Fokker F-28 (registered N485US) departing on March 22, 1992 from LaGuardia Airport (New York City) for Cleveland, Ohio. Due to a delay at the gate, a large amount of ice collected on the wings and airframe. Due to the icing and pilot error, the aircraft crashed during takeoff into Flushing Bay. 27 of the 51 people onboard were killed.
