U.S. Highway 40 Alternate (Alt US 40) is the United States highway designation for two segments of the National Pike through Maryland. The western segment begins near exit 14 on Interstate 68 and runs 31.0 miles eastward to Cumberland, where it ends at exit 44 on Interstate 68. The eastern segment runs 22.9 miles from Hagerstown to Frederick.

Branching off from US 40 just west of Frederick near the Frederick Towne Mall, Alt US 40 interchanges with Interstate 70, then crosses Catoctin Mountain at Braddock Heights. After crossing the mountain it turns northwest and runs in a straight line to Middletown, passing between the Hollow Creek Golf Club and Middletown Park. After intersecting MD 17, it runs northwest out of Middletown in a straight line before crossing South Mountain at Turner's Gap.

Southeast of Boonsboro, Alt US 40 intersects the northern terminus of MD 67 before entering the town area and intersecting the northern terminus of MD 34, the southern terminus of MD 66 and the southern terminus of MD 68 on the town's outskirts. Continuing northwest in a straight line once more, the route passes underneath Interstate 70 without any connection, a mile west of US 40's cloverleaf interchange with I-70. Entering the town of Funkstown, the route turns west onto Baltimore St., then turns north onto Frederick St. and runs into Hagerstown. After passing Hager Park in Hagerstown the route turns west again onto Baltimore St. and ends at MD 65, possibly overlapping for a short distance with the latter to meet US 40.

Alt US 40 branches from US 40 near exit 14 on Interstate 68. It runs parallel to Interstate 68 through northern Garrett County, passing through Grantsville. Through Garrett County, Alt US 40 is a two-lane road, with truck lanes on some uphill sections. After continuing into Allegany County, it descends Savage Mountain into Frostburg, where it passes through the town as Main Street. Continuing eastward, it passes through Eckhart Mines, then descends Red Hill into LaVale. Entering LaVale, Alt US 40 expands to a four-lane road near its intersection with Maryland Route 53, then narrows to a two-lane road near its intersection with Maryland Route 658.

Alt US 40 passes through the Narrows into Cumberland, where it follows Henderson Avenue and Baltimore Avenue to Exit 44 on Interstate 68, where Alt US 40 ends.

Prior to the construction of Interstate 68, U.S. Route 40 followed the route currently designated as U.S. Route 40 Alternate. The first segment of what would become Interstate 68 was built in Cumberland in the mid-1960s.[1] The freeway was extended westward through the 1970s, finally being extended to West Virginia in 1976.[2] The portions of U.S. Route 40 that were bypassed between Cumberland and Keyser's Ridge became the western segment of U.S. Route 40 Alternate.
