U.S. Route 23 runs north-south through the eastern part of the U.S. state of Kentucky, forming the northern part of Corridor B of the Appalachian Development Highway System.

- Kentucky Route 122, north of Virgie to Shelbiana
- Former U.S. Highway 23 Business through Pikeville
- Kentucky Route 1428, Allen to Prestonsburg
- Kentucky Route 321, Prestonsburg to north of Paintsville
- Kentucky Route 2565 into Louisa from the south
- Kentucky Route 3, Louisa to south of Catlettsburg

US 23 passes through the following cities and counties:
- Jenkins in Letcher County
- Pikeville in Pike County.
- Here, the road's right-of-way is in the Pikeville Cut-Through, a massive earthmoving project which rerouted the Levisa Fork River to bypass downtown Pikeville.
- Prestonsburg in Floyd County
- Paintsville, the county seat of Johnson County
- Louisa in Lawrence County
- Catlettsburg, Ashland, and Westwood in Boyd County (in which it begins to follow the Ohio River)
- Russell, Flatwoods, Raceland, Wurtland, Greenup, and South Shore in Greenup County
