I believe this image qualifies as fair use because:

the photograph succinctly illustrates the effect of Minamata disease on the human body
the historic photograph of Tomoko Uemura's crippled hand cannot be reproduced (Tomoko died in 1977)
the image is of low resolution and will not damage the copyright value of the original photograph
