Dr. Karen Scumaker (Hobbs) visits a New Zealand mental institution for a chance to interview its most enigmatic resident, serial killer Simon (Rotondo), who calls himself the Ugly. Over the course of Dr. Scumaker's interviews with Simon, she will slowly be drawn into his world and learn that sometimes the voices in a killer's head are not always his own.
