After a car chase, a firefighter is severely wounded and one of the paramedics is killed. In one of the most shocking season finales, tragedy occurs.

Maritza and Bosco are running after Buford, who was the man in charge of the meth lab that exploded, killing Maritza's sister. Cruz avoids numerous cars, but then is involved in a car crash with a motorcycle and a car, causing a huge explosion. The firefighters and paramedics quickly arrive at the scene and start classifying the injuries. A couple is trapped inside a car, and Alex goes to comfort them and try to get them out of the car. Suddenly, the explosion grows bigger and Alex is slashed by a flying piece of metal and cut in two, while Lt. Johnson is severely burned. As Kim approaches Alex, she tells her to tell her mother that death didn't hurt and dies, while Sasha, Ty and the others watch in horror. Lt. Johnson is taken to hospital, surrounded by Kim and Doc. Meanwhile, Bosco meets with Noble at a bar, without Maritza's knowing. He offers help if Noble confesses murder, but Maritza suddenly arrives after Noble calls her to check what is going on. She gets furious at Bosco for going behind her back, and sends him away. Doc and Kim are sent their results, and they get to the top of the promotion list, but overcome with guilt and grief over Alex's death, Doc doesn't accept his promotion, and Kim is named Chief of the Paramedics. Bosco arrives at the 55 building and asks Faith for help, telling her how Maritza tricked him into covering up Noble's murders only to keep him as a drug contact and asks for help, at first she refuses but then she accepts to meet him at Noble's apartment. Problem is, Maritza discovers that Faith is trying to help Bosco clear his suspect by getting Noble's gun, and gets to Noble's place to try and stop her. While holding Faith at gun point, Noble and Bosco arrives. Bosco backs Faith up and tells Maritza to lower her gun, insisting on arresting Noble, but Cruz refuses and tells Faith to turn over her gun. When Faith refuses to do so, Maritza aims at Faith, and Bosco and Faith aim at Maritza, and three shots are heard as the episode finishes.
