This template is a mini-infobox for bannered state highways.

- state
- The state that the route lies in.
- route
- The number of the parent state highway.
- banner
- The type of bannered route. Options are:
- BUS: business
- BYP: bypass
- ALTERNATE: alternate (alternate banner)
- ALT: alternate (alt banner)
- CITY: city
- LOOP: loop
- SPUR: spur
- CONN: connector
- TRUCK: truck
- OPT: optional
- HIST: historic
- location
- The location of the route (e.g. the city a business route serves, that a bypass bypasses, and so on)
- active
- The year of commissioning and decommissioning, e.g. "1935-1944". If the route is still active, write it with a dash and nothing afterward (e.g. "1945- ").
