Sisko's quest leads him to the truth about his existence, as the Pah-Wraith create an illusion that nearly convinces Sisko that he is really a mental patient in a hospital who can no longer distinguish between a story he was written about Deep Space Nine and reality.
As this occurs, Kira sets a blockade against the Romulans, and in order to ensure Jadzia's place in the Klingon afterlife of Sto-Vo-Kor, Worf, General Martok, Bashir, O'Brien, and Quark undertake a very dangerous mission to destroy Dominion shipyards.

- A sixth season episode of Buffy the Vampire Slayer uses a similar storyline of Buffy believing that she is really a mental patient in a hospital who has imagined her life as a vampire slayer. ("Normal Again")
- The same goes for an episode of Charmed where the Source of All Evil tries to convince Piper that she is a mental patient in a hospital who has imagined her life with magical powers and as a Charmed One. ("Brain Drain")
