- I'm sorry, I really didn't mean to upset you. I removed those sentences because I thought they needed a citation. You helpfully provided a citation, but I could not find the place in the book that supported those sentences. I realize I may have been too strict and that you were trying to help. I acted in haste. [edit] Hi, still plugging away (slowly) at ‘System Accident’ article, and can use your help!
First off, thank you very much for your previous contributions and your thoughtful comments. I had belatedly written comments in turn. Someone has recently deleted them, and I’m probably going to re-add shorter versions of them later. The following is generally what I said.
The most important, I’m now open to the idea that the Space Shuttle Columbia was more of a borderline case, because there were people who saw the whole narrative arc, but because of dysfunctional social factors they had to be more sure than was realistic.
I would also like to eventually add additional examples to our article, maybe Avianca 052, maybe Apollo 10. And, I’m working on some long excerpts from a 1988 book that touches on Apollo 13, which might be our best example of all--because of the movie, because it’s something people are familiar with. (The book is called FOR ALL MANKIND by Harry Hurt III, and it’s about the entire Apollo program.)
And, I must confess : ) I still like the word ‘clunky,’ but as only one of several words that can be used to describe a malfunctioning institution.
