I.
Edit PageName.

  Enter the following text at the top of the page:

{{subst:vfd}}

Be sure to include "subst:", not just {{vfd}}; this is easier on the servers. Please include "Vfd" or similar in the edit summary, and don't mark it as minor.
II.
Create its VfD subpage.

  You should see a prominent link to "this article's entry" in the new VfD text.
Follow that link (to Wikipedia:Votes for deletion/PageName) and enter this text:

{{subst:vfd2 | pg=PageName | text=Reason why the page should be deleted}} ~~~~

Include a reason after text= . Suggest what action should be taken for the article.
III.
Add a line to VfD.

  Follow   this edit link   and add a line to the bottom of the page:

{{subst:vfd3 | pg=PageName}}
