In many people's opinion, Chief Bromden does NOT have schizophrenia. There's a good possibility that the medication he was taking while in the institution was LSD as doctors were testing the effects on mental patients around that time. If you check page 13 in the book, the last line says that "it is the truth, even if it didn't happen". This is Chief's preface to telling his story about R. P. McMurphy. The ward is a microcosm of society, so by that line Chief is saying, whether or not the things he saw in the ward were real (like the fog and machinery), the metaphors they represent really DO exist in society.
