The next child of either Zeus, Poseidon or Hades to turn sixteen will make a desicion that could save or destroy the gods. This prophecy is based on an episode from Ovid's Fasti.

It is often brought up by the gods to confirm that Percy is a potential threat. Kronos tries to sway Thalia to his side because she is the next demi-god to turn sixteen.

You shall go west, and face the god who has turned,
You shall find what was stolen, and see it safely returned,
You shall be betrayed by one who calls you a friend,
And you shall fail to save what matters most, in the end.

Percy is the one this prophecy addressed. He heads west with Annabeth and Grover to find Hades. The god who has turned is Ares.
Percy discovers the lightning bolt in a backpack given to him by Ares. He makes his way to Olympus in person and returns Zeus' prized possession to him. It turns out that Ares had Hades' helm of darkness and Percy returns it to Hades' Furies after getting it back from Ares in a fight.
Percy is betrayed by Luke, who is actually working for Kronos and stole the lightning bolt of Zeus and Hades' helm of darkness during the Winter Solstice. He then summons a scorpion, which poisons Percy.
In the end, Percy failed to save Luke from turning evil. Another notion is that he had not saved his mother in the Underworld but instead left her, though fortunately she was returned.

You shall sail the iron ship with warriors of bone,

The prophecy was made for Clarisse, daughter of Ares. She sailed on an ironclad ship, with some skeletons of the Confederate Army.
She found the Golden Fleece and claimed it for herself, because it was her quest, not Percy's.
As they were fighting Polyphemeus the Cyclops, he captured her and locked her away inside of his stone cave.
Clarisse didn't get the Golden Fleece, because Tyson got it first, and because of that and her not wanting friends to help she had failed and would have to leave alone.
But, Percy changed the part of the prophecy of Clarisse failing, by being her friend (although with a grudge) and giving her the Golden Fleece to fly (she flew back in a plane) back to Camp so she would still look like the hero, because it was originally Clarisse's quest.

Campers and Hunters combined prevail,

And one shall perish by a parent's hand,

The five of the prophecy to head west to find Artemis are Percy Jackson, Zoë Nightshade, Bianca di Angelo, Grover Underwood, and Thalia.
While the five were traveling through the junkyard of the Gods, which turned out to be a desert, they got attacked by a Talos prototype, Bianca took it upon herself to save the others. She got inside of the metal giant and made it go haywire, but it fell on some electric cables, which electrocuted the entire giant, and Bianca was never found. She was most likely being electrocuted as well.
The bane of Olympus is the cow serpent Bessie, the Ophiotaurus, who continued to pop up along the journey in Hoover Dam and in San Francisco.
Throughout the quest, Percy, Grover, Thalia, Zoë, and Bianca learn to work together to find Artemis.
While battling the Titan General, Atlas, Percy finds that Artemis has taken his burden (or curse) of holding up the sky. Percy takes the curse away from Artemis and onto himself.
Zoë is the daughter of Atlas. Before they arrived on Mount Tam, Zoë was bitten by the dragon, Ladon. She fought her father, even though she had been poisoned before. He knocked her aside into the rocks, which quickened the poison's pace, leading to her death.

This prophecy was made for Annabeth.
Annabeth descended into the Labyrinth.
Nico di Angelo raised a lot of dead spirits, Ethan Nakamura was the traitor, and the lost was the god Pan.
The ghost king was not Minos as the characters first thought, but instead Nico di Angelo, son of Hades.
Daedalus, a son of Athena made one final stand and decided to finally face death.
When he died the labyrinth was destroyed.
Luke, was the love, not Percy, who has become the new body for Kronos
