Phrynopus peraccai is a species of frog in the Leptodactylidae family. It is endemic to Ecuador. Its natural habitats are subtropical or tropical moist montanes, subtropical or tropical high-altitude shrubland, and subtropical or tropical high-altitude grassland. It is threatened by habitat loss.
