State route 621 serves to provide access to the North Appalachian Experimental Watershed station from the state highway network. The Northern terminus at the experiment station join the county road network, not any other state or US highway. It follows the course of Spoon Creek valley for much of its route. Spoon Creek is a tributary of the Walhonding River.
