"My Transition" is the 24th and final episode of the fifth season of Scrubs. It first aired May 16, 2006 and is episode number 117.

The finale begins with J.D. asking Kim out on a date, suddenly he begins to panic about where to take her, wanting the perfect first date for her. He decides he wants to take her bareback horse riding across the beach. This takes a turn for the worst, as J.D. painfully learns the dangers of a man bareback riding down the beach. This gag is lifted from Donald Faison's previous series, Clueless, where he, in the role of Murray, is the bareback-rider. Fortunately, Kim's specialty is urology and she helps J.D. with his manly pains, much to his dismay. The date then moves back to the apartment, where they are interrupted by Elliot Reid (who leaps in, knowing full well Kim and J.D. could be having a romantic moment), referred to as "Elliot Reid - Moment Killer."

The next day at the hospital, Elliot plans Carla's baby shower, which is to take place in the cafeteria, while Carla invites people to feel the baby kicking. Jordan and Dr. Cox use the baby shower as an excuse to rid themselves of all their baby gear, as Jack has just completed, or is at least thought to have completed, his potty training.

J.D. attempts to ask Kim out on a second date, but is once again blocked by Elliot who has made plans to go to a Neil Diamond concert with Kim. Which leaves JD alone for the night, which he uses to call his mother. Luckily Elliot has a change of heart, and Kim arrives at J.D.'s unexpectedly and the two spend the night star gazing.

The episode begins to draw and Jordan is found to be pregnant again (despite Dr Cox's vasectomy and Jordan's fidelity), much to their anger and dismay. Dr. Cox and Jordan then duct tape the doctor who performed the vasectomy to his desk chair, forcing him to listen to the hospital's a Capella group sing the Chilli's "I Want My Baby Back Ribs" song while only repeating the "I want my baby back baby back..." part over and over. Later at night Carla and Turk go around Jordan and Dr. Cox's and discuss the pregnancy. The episode then cuts to two and a half weeks later where J.D. arrives at Kim's door with flowers, hoping for a third date. She tells him that she is pregnant.

  • As a test, this was the first and only episode of Scrubs to ever be broadcast in High Definition (HD) in the US. However, the test didn't go over well and the decision was made to continue broadcasting in 4:3 Standard Definition.[citation needed]
