Janitor starts a hospital newspaper titled The Janitorial, which he uses to torture the staff. J.D. and Turk try to prove their masculinity after Turk has a testicle removed due to testicular torsion. Elliot and Carla's patient "John" is diagnosed with breast cancer, and is too embarrassed to tell his brothers. Dr. Kelso asks Elliot to help him keep his job.
