[edit] Plot

When Carla reveals that she might not go back to work after the baby, Turk starts to perform 'off the clock' examinations to people because he is worried that he is not going to be able to earn enough money to support his new whole family. However after J.D. gives an examination to a cotton candy seller who later ends up in hospital, he has a crisis of confidence. It is later revealed that the vendor was shot. J.D. gives the $20 back to the vendor and Doug finds it in the morgue.

While Elliot treats a doctor who operates a private practice, Cox sarcastically teases her when her patient defies her treatment.

Meanwhile, a new coffee shop, "Coffeebucks" (a parody of Starbucks), opens inside Sacred Heart hospital, which proves to be very popular. One of J.D.'s former interns, Jason "Cabbage" Cabbagio, gets a job there.

The Janitor and the rest of the Hospital support staff ask Dr. Kelso for a dental plan with their salaries. When he refuses, they get employment in the coffee shop which includes a dental plan. The Janitor impresses the chain's corporate bosses, and is made manager. He then refuses to serve Kelso and tells the other employees to do the same.

After being invited by her patient to join his practice, Elliot eventually decides to accept the offer. She can still work at the hospital but she no longer is under the supervision of Dr. Kelso or Dr. Cox. Kim finds herself a new job, in Tacoma, Washington. Eventually Kelso gives the Janitor and the support staff their dental plans because the hospital has become littered with empty coffee cups.
