This produces the requested format for a normal move on Wikipedia:Requested moves. That is:

{{subst:move2|Article|New Name}} will automagically become

• Talk:Article — Article → New Name

This template is intended only to be used on, or when discussing, that page. There is no reason to use it otherwise than with subst; links to this template should be broken and replaced with substitions. Septentrionalis 18:42, 23 August 2005 (UTC)
