[edit] death date
The erroneous date has been in the article long enough to have clogged any search results quite thoroughly, but there are several reliable outside sources confirming the date of 4 October: Telegraph, Times, New York Times (needs login). I suspect the date of 9 August comes from a misreading of the date on the Telegraph obituary; 08/10/2004 means, to a British publisher, 8 October, 2004 (a Friday)—but to an American or, apparently, a Polish reader, it means 10 August, 2004 (a Tuesday). —Charles P.
