Route 122A is a southeast-northwest state highway in Massachusetts. It is an alternate route of Massachusetts Route 122. The alternate route first splits from Route 122 in Grafton and works its way northwestward until meeting Route 122 again in Rutland. Along its route, Route 122A passes through the following towns:
- Grafton
- Millbury (partially shared roadway with Route 146)
- Worcester (partially shared road with Route 146, Route 122, Route 9, and Route 12)
- Holden
- Rutland
