Maryland Route 638 is a north/south state highway located in the U.S. state of Maryland. Also known as Parkersburg Road, the route runs from Maryland Route 743 (Old National Pike) in Eckhart Mines, Allegany County north to Maryland Route 36 near Mount Savage, Allegany County.

Maryland Route 638 begins at Maryland Route 743 (Old National Pike), an old alignment of U.S. Route 40, in Eckhart Mines, Maryland. It runs northward, intersecting U.S. Route 40 Alternate a short distance from the starting point of Route 638. It then ascends Federal Hill through a residential area. The route then descends the hill, with several right-angle turns during the descent. It passes over the tracks of the Western Maryland Scenic Railroad on a narrow, one-lane bridge. The route ends at Maryland Route 36 near Slabtown, Maryland. Route 638 is a rural two-lane road with speed limit 30 mph for its entire length.
