Maryland Route 281 is a state highway that connects Elkton in Cecil County, Maryland with the Delaware state line.

From the Delaware border, Maryland 281 extends Delaware's Old Baltimore Pike to Elkton as Red Hill Road. In Elkton, the road is East Main Street and it joins with Maryland 7.
Maryland Route 781 (Delancy Road) connects Maryland 281 with U.S. Route 40/Maryland 7 in a near due-south line.
