Carter continues to work with the hypochondriac woman. Benton works with an injured convenience store clerk who insists on defending himself when his store is robbed.

Among the patients are a young male that Benton knows that lost his leg, a friend of Ross that dies suddenly while waiting for treatment, and a man who is snowed in at home and calls Greene when his wife refuses to go to the hospital to have her baby. Lewis tries to save a man that suddenly develops an aneurysm, and the patient is saved by the most unlikely person.

Corday operates to save the leg of Alison Beaumont, a firefighter, and the patient goes into a coma, causing Corday to question her own ambitions.

April Fool's Day causes the staffers to play pranks on each other; Malucci gets the worst of it when Kovac injects him with Haldol, rendering him useless for the rest of the shift and getting his hand glued to his forehead while asleep. Corday endures a particularly long surgery just to prove that she can despite her pregnancy. Greene treats three young students who have been huffing carpet cleaner. Luka treats another pregnant teenager, who wants the comfort that a child will bring her.

The two men grab Sam and make it out of the building. She becomes their hostage, along with her son, Alex, who was already in the waiting getaway van. Back in the ER, the staff try to save Jerry, and Abby collapses just outside a trauma room where Luka is being treated; he helplessly watches her place a bloody handprint on the door window just before she slumps over and falls to the floor. Ben and Gracie get out but Sam and Alex are trapped in the building.

Morris is given a paper from upstairs and goes to Pratt, who is working with Hope and Gates on a young girl high on some drug, and tells Pratt that the Medical Board suspended Pratt's license so he can't work at County anymore. Pastor Watkins surprises Pratt and the board by showing up with the congregation to speak on Pratt's behalf. Hope also comes to support Pratt. Pratt and the group are dismayed then relieved when the board comes back with their decision. Ray and Neela spend time together getting coffee, talking about work. Sam is reluctant to get involved with Ben, from the previous episode. Patients include a little boy with possible head lice, a stand up comedian diagnosed with leukemia, a man who suffered severe burns while trying to cremate his cat on the stove, a young woman high on drugs and an overweight woman in her 30s who met a man online and later in real life then tripped on a fire hydrant while running after him when he stuck her with the bill.

Sam is overworked because some nurses are sick, others on vacation, etc. Gates and Kovac treat Anna Hayes (guest star Sean Young) who is vomiting and has an altered mental status. Gates' father takes Sarah out to watch a movie and when they come back, Tony realizes that his dad has been drinking, which prompts Tony to pack his father's things and asks him to leave; they later get into a fistfight. Also, Kovac decides that he only wants to be a doctor and resigns as the E.R. chief

The ER staff and other hospital personnel arrive at a mandatory department dinner, only to find to their surprise- including Abby's- that Luka, with Hope's aid, has planned his wedding to Abby that night. After first balking at the idea, Abby agrees and the wedding takes place. When Morris points out to Hope that Luka and Abby will not be using their honeymoon suite, Hope suggests that the two of them use it. Neela suffers through a strange night, attracting the romantic interest of Gates, Ray, her supervisor Dr. Dubenko, and a young (female) medical student. Ray, frustrated by Neela's indecision in choosing between himself and Gates, gets drunk and fights Gates. Ray leaves the wedding and goes to a bar to continue drinking. Neela breaks up with Gates and calls Ray; as Ray drunkenly stumbles to answer the call in the street, he is hit by a truck.

The ER reopens with apparently very few changes made. Luka and Abby celebrate their first few days as a married couple and prepare for their honeymoon. Hope and Morris are still spending time in the Honeymoon suite, and Morris is trying desperately to keep it up, feeling that it is his only chance to keep up their relationship. Pratt is incensed by his first meeting with ICU chief Dr. Kevin Moretti, who criticizes ER procedures. Dr. Dubenko orders Dr. Crenshaw to perform an audit to cut down on post-operative infections, so Crenshaw orders Neela to move a patient from post-op to ICU before Neela feels the patient is ready. The patient crashes and suffers possible brain damage, leading Moretti to share with Neela his desire to improve the overall working of the hospital to prevent needless deaths. Neela is looking for Ray; she visits his apartment and finds that his fish are dead. Hope tells Morris not to worry about their relationship. Gates talks to a lawyer who tells him that he stands a good chance to keep custody of Sarah; Sarah tells Gates that she wants to stay with him. Luka receives a phone call from Croatia. Dr. Moretti accepts the position as ER chief.

The new Chief of Emergency Medicine Kevin Moretti (Stanley Tucci) makes an immediate impact on the ER on his first day, letting the doctors know that things are going to be different from now on; his point is especially made clear to several of the doctors, particularly Abby (whom he says botched the treatment of a woman who had come to the hospital seeking treatment). Meanwhile, Luka announces he is leaving the ER to travel to Croatia to be with his ailing father. Meanwhile, Gates loses his bid to get custody of Sarah (her grandparents arrive with legal documents demanding custody); he is left to assure a tearful Sarah that he will see her as often as he can. Neela finally receives a call from missing Ray's cell phone. She goes to see him in a hospital, only to find that he has had both of his legs amputated due to his accident. After Ray's mother takes him home to Baton Rouge, Ray's ex-girlfriend Katie tells Neela that the accident was Neela's fault. Neela is attending an anti-war rally when demonstrators begin clashing with war supporters; a smoke grenade goes off and Gates — who is trying get back with Neela — arrives just in time to be met by a stampeding crowd whilst Neela is being trampled by fleeing protesters. Pratt and Morris find themselves forced to defend their actions, while Hope prepares for a new mission. Gates struggles his new assignment, until he befriends a precocious young patient who has already diagnosed himself with an incredibly rare condition, Fazio Londe syndrome.

A patient's mother gives the ER staffers makeovers in appreciation. The ER gets a new receptionist.

The team treat a number of elderly patients brought in after the air conditioning in their nursing home breaks down, and Abby and Pratt disagree over the diagnosis of a 3 month old baby with suspected colic. Dr Moretti meets up with his ex wife, who is in town for a conference. She’s worried about their son Daniel and Moretti says he will talk to him. Morris and Pratt pass the boards and the team go out to celebrate. Later Abby wakes up in Moretti’s apartment, he tries to stop her as she leaves, obviously upset. When she gets home she finds Doris, her son’s latest babysitter on the phone to the police, concerned because by 3am Abby still hadn’t come home. Upset and a little drunk, Abby impulsively decides to go and see Luka, so she and her son head to the airport in an attempt to get a flight out to Croatia.

Abby's attitude is beginning to affect the staff. Luka returns from Croatia a day early. Neela's intern makes her proud when he deftly handles a critical situation. Tony and Julia's tryst is interrupted when Sarah needs rescuing. Morris treats a British contestant injured during a cheese rolling event, and acts as adjudicator when the contestants come to blows over which of them won the competition. Meanwhile Moretti gets an upsetting and serious phone call about his son and leaves as soon as possible to take a flight out to Providence.

Sam, Pratt and Morris treat Melissa Tanner, a 61 year old woman with health problems, and her 19 year old son Lowell following a car accident. Lowell has Down’s syndrome and had been living in a foster home since he was a baby. Melissa had been visiting him every week without her husband’s knowledge. Also, Neela and Dubenko have been at the dress rehearsal for the hospital holiday pageant and Dubenko has been bitten on the leg. Meanwhile, following the death of his father, Luka and Abby prepare to fly out to Croatia and it appears that Abby is tackling her drinking problem.

After being passed over for Chief of the ER, Pratt considers taking a position elsewhere, but reconsiders after working with Jeanie Boulet. She comes in after her son, Carlos, is admitted from an apparent head injury, and he is found to have a brain lesion -- possibly a sign of full-blown AIDS. Before they find out what's going on, he goes into convulsions. A woman soldier comes in with a broken leg, and is discovered to be pregnant. When her husband realizes that she got pregnant while separated from him when on duty in Iraq, she says her pregnancy is the result of her rape by her sergeant there. Neela's cousin is in town for a visit, and Morris and Harold compete for her affections. Neela earns an ortho rotation.

Morris tests a Taser on himself with very unpleasant results.

Abby told Luka about her drinking and infidelity. Sam's sour attitude gets her grief from both Abby and Gates, so she makes a grand gesture towards the latter. Neela's mom of four has bad news and Neela helps her deal with it. Brenner turns out to be Anspaugh's nephew. Dubenko's relationship with Skye collapses after he accuses her of having a pathological need for male attention.

When the young girl starts to bleed out, her boyfriend holds the hospital staff at gunpoint and forces them to operate in the ER.
