The image represents the icon of a saint, Cyriacus the Anchorite and is used only once in the wikipedia article Cyriacus the Anchorite. Icons are considered art for worship and the hagiographers are usually anonymous. We rarely know who made the original icon but hagiographers tend to copy from an earlier template.
