An episode of Transformers in which Optimus Prime has prostate cancer. Optimus has to use the restroom frequently, and sees blood in his urine. Dr. Ratchet diagnoses the cancer, and Optimus finally dies in the hospital, transforming into his own casket. The episode turns out to be a dramatization and ends with a public service announcement by Optimus (who is still alive).

- Some of the Autobots (Bumblebee, Ironhide, Ratchet, and Arcee) were built specifically for the Transformers segment using modified parts to make them look more "realistic." A modified RobotMasters Optimus Prime was used to portray Ironhide.
- Seth Green said on the DVD commentary for the Transformers segment that he attempted to get Peter Cullen (Prime's original voice actor) to do Prime, but he heard he took the role very seriously and declined.
- When Rachael was on top of the building she says, any questions, the 3 people down below shook their heads. The girl behind the 2 guys was the same girl in the cross between Total Request Live and C-SPAN.
