For general rules about fair use rationale requirements, see WP:FURG. For album covers, there's a handy templates that simplifies things. If you're using it in the infobox, just paste in {{album cover fur | Article = Article name | Use = Infobox}} (Where article name is the name of the article you're putting it in.)
