Isthmohyla graceae is a species of frog in the Hylidae family. It is found in Panama and possibly Costa Rica. Its natural habitats are subtropical or tropical moist montanes, rivers, freshwater marshes, and intermittent freshwater marches. It is threatened by habitat loss.
