All fields other than name are optional.

[edit] Usage

width
This is used to override the default size. If you need to do this (and please don't unless you know exactly what you're doing and why), it should be set to auto or some other CSS-compatible measurement. The default width is 23em.
name
This is for the name of the mobile suit. You can skip this variable if the name of the article is the same as the name of the mobile suit.
image
This is for an image of the mobile suit. Don't use the frame or thumb options, but do put the image in proper brackets. The template will automatically replace the image with Wikipe-tan outside of the article space, for fair-use reasons.
caption
This adds a caption below the image.
series
This is for the series (or game, or movie, or whatever) the mobile suit appears in. If the mobile suit appears in a main work and then associated works (say, Char's Counterattack and a cameo appearance in a game set in the same period), just link the main work. If it appears in more than one series, don't use this variable.
first
This is for the first appearance of the mobile suit. Mention a specific episode or manga chapter, if applicable.
designer
This is for the real world artist who designed the mobile suit.
faction
This is for the associated faction or factions. Don't cram too much into this field; with the Zaku II, it's fine to just have Principality of Zeon despite the intermittent use by Neo Zeon and the Earth Fed and others, but the GM or the various unique suits that get stolen back and forth should probably have no faction.
pilot
If a single pilot is associated with this mobile suit, add it here. This is best for one-shot custom mobile suits or mobile suits in some of the spinoff series (like G Gundam or Gundam Wing).
class
This is for the class of weapon, be it mobile pod, mobile suit, or mobile armor. If you just type "mobile pod", "mobile armor", or "mobile suit" (without the quotes), it will automatically link to the relevant section of Mobile weapons. Otherwise it's like any other text-entry field.
height
This is for the height in meters.
