In this episode, Mr. Mackey gives a drug & alcohol prevention lecture, emphasizing the negative effects of smoking, drinking, marijuana, and LSD. He passes a sample of marijuana around the class so that the children can learn its smell, but it is never passed back up to the front. (It turns out that their teacher, Mr. Garrison, was the one that took the marijuana.) As a result, Mackey is fired. Meanwhile, Kyle invites Stan, Cartman and Kenny to Ike's bris, but when they discover what it actually is (or as they construe it: party where they "chop off his penis"), Kyle is shocked and tries to find a way to hide his brother from his parents. After being kicked out of his house by his landlord following the drug incident, Mr. Mackey ends up homeless, and is given a spliff by a hobo. The next day, Mr. Mackey meets two hippies in the woods and has some of their LSD. Soon enough, Mr. Mackey becomes a drug-addled hippie, meets a hippie woman and goes to fingerpaint at her house - before long, they decide to get married and have a honeymoon in India.
Meanwhile, Kyle puts Ike on a train to Nebraska and makes an Ike-style voodo doll in an attempt to not arouse his parents' suspicions. This backfires when the doll is eaten by a dog, and both the dog and doll are run over by a truck and explodes in front of the Broflovskis' house. It is at the funeral that Kyle finds out that Ike is not his biological brother, but was adopted from Canada. Upon discovering this, Kyle decides that Ike is not his "real brother" and he no longer cares about him; he then tells his parents that Ike is not really dead, and his parents retrieve him. While on honeymoon, Mr Mackey is captured by The A-Team, and his former employers, along with Jimbo, say that they should have helped him with his drug problem rather than firing him. In rehab at the Betty Ford Clinic, he is "cured" of his addiction to drugs.
The day of the bris arrives, and Kyle is grounded, sulking in his room. When the Mohel arrives to perform the bris, Ike flees to Kyle's room in terror. Seeing Ike in distress prompts a change of heart in Kyle, and he defends his brother fiercely before it is explained to him what a circumcision actually is, "snipping the penis a little to make it look bigger", at which point Stan and Cartman decide they want to be circumcised too. The episode ends with Mr. Mackey telling the class again about the evils of drugs; all is back to "normal".

At the graveyard shortly after Ike's funeral, he falls into a grave and is crushed by a tombstone, followed by another short funeral. Kyle says his line like he doesn't even care. This might be a reference to one of the death scenes in the first part of the movie Creepshow. He is seen later in the classroom after the bris, however after the scene of Mr. Garrison talking, Kenny's desk is empty again.

- In the original airing, Mr. Mackey tells off Jimbo, stating that he's not going to listen to his "authoritative, right-wing bullshit!" with the word "bullshit" uncensored (marking the first time that "shit" has been spoken on South Park without getting bleeped out[citation needed] ). All reruns and the Season 2 DVD version have the word bleeped out, but airings on the Canadian cable channel The Comedy Network have the word uncensored.[citation needed]
- In syndication, Cartman's line "the fireman spitting when you rub its helmet" (in reference to why a man's penis shouldn't be cut off) was cut. It is not cut from the reruns on Comedy Central.[citation needed]

- The pronunciation of "bris" marks Kyle's family as Ashkenazi Jews.[citation needed]
- The alphabet panel above the blackboard contains the rearranged letters "DiOsMiOhAnMaTaDoHaKeNnYbAsTaRdOs..." which is the alternate-cased, space-removed, no punctuation Spanish phrase for "Oh my God, they killed Kenny, you bastards" which is the popular catchphrase shared by Stan and Kyle upon witnessing Kenny's death.
- The Israeli Psytrance group Alien Project has a song entitled "Who has the Marijauna" on their "Don't Worry, Be Groovy" album that centers around samples of Mr. Mackey from this episode.
- The second half of the episode begins with a short musical introduction that resembles the theme song for the television show, Seinfeld.
