In the uppermost image both testicles are missing from the scrotum (empty scrotum/cryptorchidism). In the middle image right testicle is missing (monorchism/cryptorchidism). In the lowest image both testicles are in the scrotum. Cryptorchidism is pretty common condition and it is usually cured in childhood, if possible. Images of missing testicles are taken by pushing the testicles gently into the groin.
