State Road 686 is an east-west route in Pinellas County originally running east from Belleair Beach, through Largo to the eastern side of the county, then southeasterly to northeastern St. Petersburg, Florida; it now ends at Alt US 19 in Largo. It connects with US 19 and Interstate 275. It then meanders further to the southeast, and ends at an intersection with 4th Street (State Road 687) and Gandy Boulevard, State Road 694.

The highway changes its name several times as it heads east across Pinellas County. From Gulf Boulevard in Belleair Beach, through Belleair Bluffs to Indian Rocks Road, the road is called the Belleair Causeway. In Largo, it becomes West Bay Drive. West Bay Drive is historically Largo's "Main Street". Between Gulf Boulevard and Clearwater-Largo Road, the road is also known as Pinellas County Road 416. At the intersection with Missouri Avenue/Seminole Boulevard, it becomes East Bay Drive. It remains East Bay Drive until it crosses US 19, where it becomes Roosevelt Boulevard, named for President Franklin Delano Roosevelt.

As Roosevelt Boulevard, it continues east through and out of Largo. It turns southeast and then south to merge with Ulmerton Road, State Road 688 for just under a mile. Leaving Ulmerton, it again turns to the southeast, enters St. Petersburg and runs southeast to the intersection with 4th Street and Gandy Boulevard.
