The DASS, or Depression, Anxiety and Stress Scale, is a public domain psychological assesment test to measure the constructs outlined in its name. Its full form consists of 42 self-report items (14 items in each subset), though a shorter 21-item version has been made available in the last year. The test itself is available for free, though the manual costs AU$55. (Cheap, by psychological tests standards)
