Simple use this formula:
{{comicbookdb|type=creator|id=1147|title=Charlie Adlard}}
As shown here: Charlie Adlard.
If the entry isn't disambiguated (like the above example) you can trim it down to:
{{comicbookdb|creator|1147}}
Specifying that they are type, id and title isn't actually required but doing it in long form might be helpful to future editors. (Emperor 15:46, 8 October 2007 (UTC))
