Presumably a much-reduced scan of the cover; it is certainly an accurate, reduced reproduction, but who made the scan seems to be lost in history (and irrelevant, since there is no copyrightable creativity in a reduced scan of a 2-dimensional document).

Picture of first issue seems a particularly appropriate illustration. The magazine is defunct (last issue was in 1995), so there is no obvious place to request explicit permission to use this.
