Urban Legends: Bloody Mary is a 2005 horror film starring Kate Mara, Robert Vito, Tina Lifford, Ed Marinaro, Michael Coe, Lillith Fields, Nancy Everhard, Audra Lea Keener, Don Shanks, and Jeff Olson. It went straight to DVD in mid-2005.
Urban Legends: Bloody Mary starts off in 1969 at a high school in Utah, where the captain of the football team and his friends drug two of their dates. The third girl, Mary, tries to escape but hits her head. She is accidentally killed and her body stuffed in a trunk. Fast-forward to present day, and some girls discuss the legend of the missing girl and then also the infamous "Bloody Mary" game. Strange things happen, girls disappear, and apparently the death of choice is that based on urban legends. The only question that remains is: why is Mary targeting these particular girls?
The movie was filmed in Salt Lake City, Utah.
This movie is the third film, the second "non-sequel" sequel in the series, that began with Urban Legend and Urban Legends: Final Cut. Further removed from the original than Final Cut was, this sequel contains no characters from either of the two previous films, though mentions are made of the murders at the schools from the previous movies. Bloody Mary even abandons the slasher element of the other two movies in favor of a supernatural element.

The movie begins showing a high school in Salt Lake City, Utah in 1969 at a senior prom. At it, three girls, Mary Banner (Lillith Fields), Grace Taylor (Tina Lifford), and Gina Lotnick (Haley McCormick) show up at the prom with their dates, jocks from the football team. While dancing, one of the jocks gets the girls some drinks, and drugs them. While Mary is dancing with a jock, she finds her friends are gone. She goes looking for them, and finds them drugged, being stuffed into a trunk by the football team, and a popular girl in a petty revenge tactic for stealing her boyfriend. Mary then runs through the school from her "boyfriend" and hides in the school storage closet. However, her date finds her and attempts to get her to come with him, but she bites his arm prompting him to hit her, causing her to hit her forehead against a nearby table and lose consciousness, appearing to be dead. In panic the jock puts Mary in a trunk and locks it. The viewer is led to believe that this is the cause of her death, however we later discover that Mary was still alive and only after waking up in the locked trunk and failing to escape she dies. The body isn't recovered for 30 years.
At a sleepover, Samantha Owens (Kate Mara) and her friends are discussing that story as an urban legend. They then begin talking about Bloody Mary, and about the various things she would do once summoned (sucking her victim into the mirror for instance), and after a little more fun is had, Samantha says "Bloody Mary" three times, summoning the ghostly demon. And, as it turns out, "Mary" starts to brutally murder the children of all those involved in her death.
