Screen capture from the game Wolfenstein 3D used to illustrate the protagonist William "B.J." Blazkowicz. This use is believed to qualify as Fair Use as it is of low resolution, does not have any commercial value, and does not interfere with the commercial value of the game (which, coming out in 1992, it doesn't have much of anyway at this point). Video game characters can only be illustrated by non-free images; the use to illustrate the character's article helps explain his in-universe characterization.
