1989 CONCACAF Championship qualification

A total of 16 CONCACAF teams entered the competition. However, FIFA rejected the entry of Belize due to debts to FIFA.
There would be two rounds of play:
- First Round: Canada, Honduras, El Salvador, USA and Mexico, the five best ranked teams according to FIFA, received byes and advanced to the Second Round directly. The remaining 10 teams were paired up to play knockout matches on a home-and-away basis. The winners would advance to the Second Round.
- Second Round: The 10 teams were paired up to play knockout matches on a home-and-away basis. The winners would advance to the tournament.

Trinidad and Tobago advanced to the Second Round by the aggregate score of 5-0.

May 15, 1988, San Marcos, Guatemala - Guatemala 1 - 1 Cuba
Guatemala advanced to the Second Round by the aggregate score of 2-1.
May 12, 1988, Kingston, Jamaica - Jamaica 1 - 0 Puerto Rico
May 29, 1988, San Juan, Puerto Rico - Puerto Rico 1 - 2 Jamaica
Jamaica advanced to the Second Round by the aggregate score of 3-1.
June 19, 1988, St. John's, Antigua & Barbuda - Antigua and Barbuda 0 - 1 Netherlands Antilles
July 29, 1988, Willemstad, Netherlands Antilles - Netherlands Antilles 3 - 1 (aet) Antigua and Barbuda
Netherlands Antilles advanced to the Second Round by the aggregate score of 4-1.

Costa Rica advanced to the Second Round by the aggregate score of 3-1.

United States advanced to the Final Round by the tournament score of 5-1.

October 15, 1988, Vancouver, Canada - Canada 3 - 2 Guatemala
The aggregate score was tied 3-3, and Guatemala advanced to the tournament due to the away goals rule.
October 30, 1988, Port of Spain, Trinidad and Tobago - Trinidad and Tobago 0 - 0 Honduras
November 13, 1988, Tegucigalpa, Honduras - Honduras 1 - 1 Trinidad and Tobago
The aggregate score was tied 1-1, and Trinidad and Tobago advanced to the tournament due to the away goals rule.
October 1, 1988, Willemstad, Netherlands Antilles - Netherlands Antilles 0 - 1 El Salvador
October 16, 1988, San Salvador, El Salvador - El Salvador 5 - 1 Netherlands Antilles
El Salvador advanced to the tournament by the aggregate score of 6-1.
Mexico was disqualified for fielding over-aged players in a youth tournament, so Costa Rica advanced to the tournament automatically.
