This is a Vietnamese name; the family name is {{{1}}}, but is often simplified as {{{2}}} in English-language text. According to Vietnamese custom, this person properly should be referred to by the given name {{{3}}}.

This template is for explaining to Western readers which part of a Vietnamese name is the family name, which spelling is the actually correct Vietnamese spelling (which may use diacritics not present in English, and English language sources often drop them), and which name part is the preferred given name. Like East Asian names, the family name is placed before the given name, but unlike East Asian practice, the given name, especially the last one, is the primary form of address, even in formal speech or writing, thus the article may properly refer to its subject by that given name.

which renders as:

This is a Vietnamese name; the family name is Ngô, but is often simplified as Ngo in English-language text. According to Vietnamese custom, this person properly should be referred to by the given name Diệm.

This template should not be used at the same time as Template:Non-Germanic van name even if both seem to apply. For example, the article on Nguyễn Văn Cốc would use {{Vietnamese name|Nguyễn|Nguyen|Cốc}} but not {{Non-Germanic van name|Nguyễn|Van Coc}}, because the second template would not add in any way to disambiguating the surname from the rest of the name.
