Urethrostomy

PERINEAL URETHROSTOMY IN THE MALE CAT
History: For many years perineal urethrostomy has been used in cattle, sheep and goats, especially young males that have been castrated at a young age, for obstruction by uroliths. However, the anatomy of the male cat is quite different and the urethra is very small in diameter. Perineal urethrostomy in the male cat was developed in i962 and published in the Journal of The American Veterinary Medical Association in 1963 (1). It was modified in 1967 (2), and further modified in 1971 (3).
Technique: Since many cats are recovering from uremia, preferred anesthesia is light anesthesia or sedation plus epidural anesthesia. The penis is amputated at its root, and the remaining pelvic urethra is preserved in its entirety. A small circular hole is cut in the skin and the edges of the opening of the pelvic urethra are sutured to the rim of the skin opening using very fine, 7-0, Teflon-coated Dacron which causes the least inflammatory reaction. Cats rarely disturb these sutures. The result is relatively large urethral orifice, much wider than the penile urethra, facilitating passage of obstructions. A catheter is inserted and the bladder filled and emptied repeatedly to flush out any crystals or small stones. Post-operatively, anti-inflammatory steroid and antibiotic ointment is applied to the operative site twice daily. The above ointment is dispensed for the owner to apply twice daily. Sutures are removed in ten days. If there are any signs of stricture, the opening is very gently dilated daily for a few days.
