Triple-elimination tournament

A triple-elimination tournament is a competition in which each player or team gets an allowance of two losses before the third loss eliminates them.
If someone knows a charting method for these it would be most useful. Otherwise a description of various table methods would be good. The structure below was taken from the double-elimination page.
