The season 4 premiere starts where the season 3 finale left off: Nathan jumping into the lake to save his uncle Cooper, and Haley screaming for help in the bridge, right after their wedding. Lucas and Karen, who Nathan had called in the previous episode to take Haley's purse (in which their plane tickets were) to them, arrive in their car, and Haley waves for them to stop. She tells them what has happened, and Lucas doesn't think twice before jumping into the lake to save a body that is floating face-down; we discover afterwards that it is Cooper, but a few seconds later, Lucas sees Nathan and Rachel laying on the shore of the lake.
In the hospital, Lucas and Haley wait for Nathan to wake up; when he does, Haley stays with him while Lucas goes back to the waiting room to talk to Peyton. Brooke happens to come out of the elevator while Lucas and Peyton are hugging, and she consequently changes her path towards Rachel's room. Brooke looks at her for a moment before turning her back, but then Rachel wakes up and calls her a slut, and they have a jokingly offensive conversation; Rachel later tells her that she lied when she told Cooper that she was pregnant.
Lucas and Brooke talk in the harbor and Lucas confronts her about the pregnancy test he found in her purse; Brooke tells him that it isn't hers and she is definitely not pregnant. Later in the episode, she visits Lucas's home and finds Peyton in his bedroom; Brooke tells her to go away so she can talk to his boyfriend alone, and she breaks up with him. Soon after, she visits Haley's house and asks her to accompany her somewhere; they go to the hospital, where we find out that Brooke is actually pregnant.
Peyton burns pictures of her and Brooke together while she remembers when she kissed Lucas during the Jimmy Edwards incident in episode 16 of season 3, and the moment Brooke told her their friendship was over because Peyton had feelings for Lucas. Peyton looks around Ellie's old record cases and finds a note in one of them, which tells her that she has a half-brother, Derek, son of her biological father, who she has never met.
Meanwhile, Dan goes back to the beach house and discovers that someone painted murderer in the wall of his living-room. Keith as a child appears and tells him that he is not the only one who knows Dan killed him; the police arrives soon afterward, but it's only to notify him about his son having had an accident and being in the hospital. When he's there and finds a moment alone with Deb, he threatens her. Because of this, Deb looks desperately for her pills and starts abusing them again.
After Karen checks that she is definitely pregnant with Keith's child, Dan visits her and offers his help with this baby, which Karen rejects.
Cooper's heart stops for a moment, but the doctors save him; however, he doesn't wake up for the rest of the episode. A policeman visits his room while Rachel is there and tells her that she better hope for him to wake up and say otherwise, because he has witnesses of Rachel's scene in the wedding and a theory on how the limo went off the bridge. Besides this, Rachel invites Brooke to move in her house now that she doesn't have any other place to live in; Brooke accepts. Mouth visits her and Rachel asks him whether he thinks people change or not; he tells her that people don't change, not because they can't, but because "it's easier not to". Haley visits her as well, but for completely different reasons; she blames Rachel for everything that happened in the bridge.
Nathan keeps having flashbacks to what happened in the lake; one of them involves losing the bracelet he gave to Haley on their first date, and another one shows Cooper pushing him away when he tries to save him.
The episode ends with Peyton calling her half-brother but hanging up after hearing his voice, and Lucas hugging her near the door of her bedroom. Her webcam records this, and we see someone whose username is "watchmewatchu", who Peyton thought was Ellie when she first met her, IMing her.
