Paige is feeling out of place among the happy married Phoebe and Cole along with Piper and Leo. The situation gets worse when Paige becomes convinced Cole's still a demon, while he secretly plots to impregnate Phoebe with his evil spawn.

- Power Brokers: Power brokers are demons who make a business in trading magical powers. Sometimes they use people to store powers until they find a buyer so the powers can't be stolen. Human beings possessed with a demonic power will become confused then frightened, paranoid, violent, demonic and will ultimately die if the power isn't withdrawn. The power can be pulled out with a potion or by a power broker.

- This episode's title is a play on words to the phrase the fifth wheel, or when one person proves to be one too many.
- Although this is the first episode to show the long running character Elise Rothman, the actress who plays her (Rebecca Balding) previously appeared on the Charmed series in the episode The Fourth Sister as Aunt Jackie.
- Phoebe gets an advice column job called "Ask Phoebe" in this episode.
- When Phoebe calls Piper at the club, Piper is "doing the deed" with Leo. Shortly after we see Paige call for help after her innocent attacked her. Assuming that Piper and Leo were still making love how would they have gotten dressed fast enough to orb to Paige only seconds after she called?
