Peter Hertz, a character from Puppet Master III, tells a woman, Maclain, the history of the living puppets, not knowing she wants to destroy them.
The film uses archived footage to place the history of the puppets and Toulon in chronological order.

- This film has no new cast except for Jacob Witkin and Kate Orsini. All the others scenes are from archive footage from the other Puppet Master films.
- The character Peter Hertz was the boy from Puppet Master III that escaped from Germany with Andre Toulon.
